Otitis Media - What? Why? And How?

The ear, in the world of medicine, is not just the ear that we commonly refer to. The ear is quite a complex structure and consists of 3 parts: the outer ear, the middle ear and the inner ear. Otitis media, as the name suggests, refers to infection in the middle ear.

This is the basic mechanism of middle ear infection. There are many reasons for the Eustachian tube to become blocked. One of the commonest being an upper respiratory infection or common cold, which causes the Eustachian tube to swell up and block. This is particularly common in children as their Eustachian tubes are smaller and not yet fully developed. The other less common cause being a tumor growing around the opening of the Eustachian tube in the throat, causing the tube to obstruct. This is particularly common in the middle age or the elderly population, where tumor of the throat is prevalent. Having said that, very often, an ear infection occurs 'out of the blue' for no apparent reason.

What are the symptoms of a middle ear infection? Earache is the most common complaints. It is often described as a deep ache or pressure, sometimes bad enough to disrupt sleep. Dulled hearing and deafness are also common symptoms; they usually last for several days. Fever is also common, but not very 'specific' as other infections can also cause fever. Young children with middle ear infections may also vomit and tend to look very ill. Babies cannot point to their pain but if they are irritable, feel hot and crying all the time, a middle ear infection should always be suspected. Discharge is a common symptom too. As the infection progresses, pressure begins to build up in the middle ear. When it is too high, the eardrum 'pops', allowing its content (usually pus) to flow out from the middle ear.

What is the treatment for a middle ear infection? The aim of treatment is to kill the bugs responsible, to control the pain and to control the fever (to bring the high temperature down to normal). Antibiotics are commonly given to kill the germs responsible, the commonest being Amoxicillin. Pain killers (or analgesics) are given to control the pain, panadol is usually sufficient but some people may need stronger pain killers. Lastly, panadol can also do the job in lowering the high temperature as well. The course of the disease is usually between 7 to 14 days.

How does otitis media occur? The middle ear, which is the space behind the eardrum, is normally filled with air. A tube normally connects the middle ear to the throat. This tube is called auditory tube or the Eustachian tube (Eustachian was a chap who discovered the tube hundreds of years ago). The function of the tube is to normalize or equalize the pressure between the middle ear and the atmosphere. When blocked, pressure in the middle ear decreases, causing mucus (or fluid) to fill up the space. The mucus may then become contaminated by bacteria and viruses, thus allowing infection to occur.

If our meds really worked, if they really "fixed us" so that we could lead lives filled with peace, energy, purpose, and a guaranteed good nights sleep, don't you think we'd be taking them?

Are You Helping by Asking, "Did You Take Your Meds?"

"Fixing us" is, of course, what our loving family would like to do. In their mind there is a logical correlation between "taking your meds" and leading a peaceful, productive, non-combative life. It is understandable that they fear the phone call telling them we've been arrested or are in the mental ward of a hospital or have had a car accident because in our manic state, we thought we were indestructible. Of course they want us to take our meds! It's their only hope.

It's hard as a parent to stand back and watch the destructive behavior of our children. I am a parent, and I was a fanatic when I thought anything was threatening my daughter. Fortunately, she didn't inherit my bipolar genes. I didn't have to watch any wild, destructive behavior in her that my parents had to watch in me. For those of us who have a mental illness like depression or bipolar disorder, we need to give our family members some slack. Most of them do mean well when they ask, "Did you take your meds?" But if you're a family member reading this, realize that such a question only frustrates and antagonizes the person suffering from a mental disorder.

Sometimes its appropriate and helpful to ask a family member or friend, Did you take your meds? My husband, Adrian, for example, is forgetful and wants to be reminded when he needs to take any medication. I probably ask him three or four times a day if hes taken one of his numerous prescription drugs, and hes happy to get the extra help.

The key to being helpful rather than hurtful is to assist your loved one in ways they have asked you to rather than assuming a parental role. How can I be helpful? is a better question than Did you take your meds?

One parent I know, who has lived through years of trying to "fix" his daughter with policing so that she would "take her meds," has come to a more peaceful place. "It's her life," he says. "She has to want to get better for herself, not for me. I no longer ask her if she's taking her medication or doing the things she needs to in order to stay healthy. I stay out of it." He added that he is there for his daughter if she needs him and asks for the help, but that's it.

For those of us who have a mental illness, however, like depression or bipolar disorder, we may not appreciate being asked, Did you take your meds? This issue was discussed at a bipolar support group meeting I attended, and most people felt such a question was not helpful, especially since the meds are anything but a cure-all for our problems.

It Remains Illegal to Promote Off-Label Use

Lastly, the FDA recommends that a journal reprint or copy also should carry a prominently displayed statement disclosing the following:

The uses described in the information have not been approved or cleared by FDA, as applicable to the described drug or medical device,

Any significant risks or safety concerns the manufacturer knows about concerning the unapproved use that are not discussed in the journal article or reference text.

The guidelines also suggest that the articles should not be edited, abridged, marked, or highlighted in any way, and that reprints or copies should be accompanied by the FDA-approved labeling and an extensive and comprehensive bibliography. The articles should not be physically attached to any promotional material by sales representatives, nor should they constitute a subject of discussion during sales presentations. Similarly, sales representatives should not distribute these reprints at conferences.

In general, the FDA approves drugs only for certain indications, and this information is contained in the FDA label for the product. However, there is nothing illegal about healthcare professionals prescribing FDA-approved drugs for non-indicated uses, such as administering an anti-epileptic drug to treat bipolar disorder, for instance. Indeed, it is estimated that around 20 percent of all drugs are prescribed for off-label uses, particularly in the psychiatric arena. However, it is illegal for manufacturers to actively promote drugs for non-indicated uses.

The new guidance recommends that individuals (such as drug company representatives) who disseminate literature in support of off-label use, use only letters to the editor, publication abstracts, and reports of Phase 1 trials in healthy subjects from peer-reviewed journals with recognized editorial boards. It also allows for reference publications that contain little or no substantive discussion of the relevant investigation or data. The publications should be generally available in bookstores or other independent distribution channels where medical textbooks are sold, according to the guidelines.

In the end, it is the doctors and healthcare practitioners who must decide on how to use a drug for a given condition. If an FDA-approved anti-epileptic drug has been shown to significantly improve a bipolar disorder in independent and peer-reviewed studies, then doctors and healthcare practitioners are the ones who are going to be in the best, and only, position to determine how best to treat the patient, on a case-by-case basis. With these guidelines in place, the promotion or advocacy of off-label use by drug-or medical device-companies is likely to be severely restricted.

Given that healthcare professionals can legally prescribe drugs for off-label use, the FDA guidelines represent a significant advance in this whole area. I find it puzzling to read comments in the press and in online blogs about how this is going to open up the area of unapproved, off-label use and thereby put the public's safety as risk because the FDA is now going to completely endorse publications promoting such use. If anything, this proposal sets very strict and sensible boundaries for promoting off-label use. As Randall Lutter, an FDA deputy commissioner, said in a news release referring to the publications for off-label use, they "can contribute to the practice of medicine and may even constitute a medically recognized standard of care."

One side effect of the media fuss was that Democratic Rep. Henry Waxman of California wrote to the FDA suggesting that it rethink the guidance. He said he was worried that it would create a large loophole in the laws against off-label promotion. Actually, it tightens loopholes.

The whole idea is that while the FDA recognizes that off-label use is common, and indeed quite legal, drug companies need strict guidance on what they disseminate regarding those uses. Contrary to what some of those news reports suggest, the FDA is not opening the door for drug companies to promote off-label uses of drugs. Instead, it puts very precise rules and standards in place for what the companies can use as supporting evidence, such as peer-reviewed publications.

Finally, it should be recognized that it is illegal for a drug company to promote off-label use for a drug or device, that constitutes a "new use" for that drug or device. In some cases these decisions can result in a product being deemed misbranded or adulterated under the federal Food, Drug and Cosmetic Act.

Before you meet with the physician, you should compile a list of questions. They may be something like:

(1) How long has he been in practice?

(2) Is he Board certified?

(3) What is his stance on patients being involved in their own care?

(4) Is he open to patients asking questions about their own case, and will he answer openly and honestly?

(5) What are his office policies on after-hour calls?

(6) How long does he allow for patient appointments?

(7) When you need to be seen, what is the normal time frame it takes to get an appointment?

(8) What are his payment policies? Does his office accept your insurance?

A doctor should be like an old friend. Someone we feel comfortable with knowing our most personal secrets; such as, how much alcohol we really consume. How many cigarettes do we generally smoke a day? And are we really trying to shed those pounds he's been insistent on us dropping? We need to feel free to be honest, even honest about how regularly we really take that medication he prescribed for us. But the truth is, a lot of us don't feel comfortable with our physicians. And we don't know how to communicate how we feel when it comes to our concerns about our healthcare. For some, it's a lack of trust. For others, a lack of respect. And still others, their personalities simply don't mesh.

Instead, we pick a name out of the phone book and trust that he is not only knowledgeable in the particular field of medicine we are seeking, but that he has our best interest at heart.

There comes a time in each of our lives when we all need a doctor, whether it is for an inconsequential illness, such as a head cold, or for a more serious reason that may be life threatening. Whatever the reason, we all need to have a doctor whom we not only have a good rapport with, but one we trust with our lives.

When our health is compromised, we want someone who will give us the utmost care, attention, and consideration. We want someone to have our best interest at heart. And we want our concerns to be validated. If you don't get a good first impression, you might want to keep looking; interview more physicians. Not every doctor is for every person. Just like buying a new car, or even a new pair of shoes, we shop around. What might be a good fit for one person, may not be a good fit for you. If you do your research and plan ahead, the next time you need a doctor, you may find that the healing process is much easier to handle if you already have an idea of who will be treating you.

It can be frustrating trying to get your feelings across to your physician. Some people feel uncomfortable or out of place speaking their mind to someone with a plaque hanging on the wall; after all, he's the doctor and you're just the patient. And so, they keep showing up to their appointments and walking out of the office feeling more frustrated than their last visit.

There are some tips to finding the right doctor for you, and to start, like with trying to land a wanted job, it requires an interview process. An interview with a physician should be done prior to needing his services. You should schedule an appointment for a time when you are not ill, and let the scheduler know that you are seeking a new physician and would like to come in and just talk with him and ask some questions. A lot of times, these types of appointments are scheduled at the end of the physician's day, in their last time slot. If the scheduler tells you that the doctor does not see possible patients for interviews, that might warrant a red flag that you should pay attention to. Most doctors will meet with possible patients to answer any questions they might have, prior to needing their services, and if they don't, you may want to keep looking.

The quinolones have evolved from drugs used solely for the treatment of urinary tract infections to molecules with potent activity against a wide spectrum of bacterial pathogens and clinical utility in many indications throughout body tissues and fluids. Progressive modifications in molecular configuration have resulted in improved breadth and potency of activity and pharmacokinetics, which have identified those agents fit to survive in today's therapeutic environment.

Ciprofloxacin and ofloxacin are the most widely used second-generation fluoroquinolones because of their availability in oral and intravenous formulations and their broad set of FDA-approved indications.

The first-generation agents ("quinolones") had poor distribution into the body tissues and limited activity. They were used mainly for treatment of urinary tract infections. The early quinolones include: cinoxacin, nalidixic acid and oxolinic acid.

History of Quinolone Antibiotics

The fluoroquinolone family is a relatively new group of antibiotics. They were first introduced in 1986, but they are really modified quinolones. The parent of the group is nalidixic acid. The majority of quinolones belong to a subgroup called fluoroquinolones, which have a fluoro group attached the central ring system. Both terms are used to describe antibiotics in this class.

Second-generation fluoroquinolones have significantly increased antibacterial activity. These antibacterial agents have increased gram-negative activity, as well as some gram-positive and atypical pathogen activity. Compared with earlier quinolones, these drugs have broader clinical use in the treatment of complicated urinary tract infections and pyelonephritis, sexually transmitted diseases, pneumonias and skin infections.

The third-generation fluoroquinolones (levofloxacin, gatifloxacin, moxifloxacin, sparfloxacin, trovafloxacin) have expanded activity against gram-positive bacteria and atypical pathogens, such as Mycoplasma pneumoniae and Chlamydia pneumoniae. Third-generation antibiotics are widely used in the treatment of community-acquired pneumonia, acute sinusitis and acute exacerbations of chronic bronchitis.

The fluoroquinolones are classified into generations based on their antibacterial spectrum, with earlier generation quinolones more effective against Gram-negative bacteria and the spectrum of activity expanding to include more Gram-positive bacteria with the later generations.

I feel so strongly about nasal irrigation that I believe it must be tried first before considering nose or sinus surgery (unless your situation demands it).

Allergies can lead to enlarged turbinate tissue that can block nasal breathing, so effective treatment for allergies can actually reduce the size of the turbinates, an effect that can be very beneficial. (Please note: the turbinates are very complex structures in the nose with the primary turbinates the size of a finger and they play key roles in heating, humidifying, and filtering air, directing and detecting airflow, and providing 50% of resistance in overall airflow to the lungs.) This treatment might consist of allergy medicine and injections. Allergy injections, for example, have been demonstrated in medical literature to improve the immune system and mucociliary clearance functioning. These are generally administered for 3-5 years for optimal benefit.

Principle 2 is maintain good blood supply to the nose. Exercise is a strategy that most people are familiar with and can help improve nasal functioning. Exercise increases blood flow throughout the body, while increasing serotonin reuptake inhibitors that will physically decrease the likelihood someone will develop depression. When I run outside (or walk or do any physical activity), I notice that I can breathe better through my nose. Empty nose sufferers tend to particularly benefit from good blood supply to their nose, sometimes finding inversion techniques to be helpful. An inversion technique consists of having the head lower than the rest of the body to increase blood supply to the head and remaining nose tissue (the turbinates). You can purchase an inversion table to do this technique.

Nasal irrigation has become popularized in 2007 by Oprah Winfrey, as she had Dr. Mehmet Oz introduce the neti pot to viewers. Dr. Oz suggests water used for irrigation should be warm and it must include salt; without salt in the mixture that mimics the natural concentration of salt in the body, the water would irritate delicate nasal membranes. He also points out how many ear, nose and throat (ENT) specialists recognize the value in doing irrigation, which can be more effective than drugs for treating nasal congestion, stuffiness, or allergies, for example, as it involves directly cleaning out the nose and sinuses. Have you noticed the proliferation of irrigation products in your local drugstore compared to even just 10 years ago? The number of products has increased significantly, because there is much value in cleaning out your nose with salt water. I wish nasal irrigation would be tantamount to and as well understood as washing dirty hands!
